- Percussion and Palpation Tests: Tapping gently on the suspected tooth may help to localize pain; palpation around the gum may indicate periapical inflammation.
- Thermal Tests: Cold and heat tests can be used in determining pulp vitality. A sharp, lingering response to cold may indicate irreversible pulpitis, whereas no response may indicate necrosis.
- Electrical pulp testing (EPT): This device is used to evaluate pulp vitality by the application of a low electric current. It is particularly useful when multiple teeth are being evaluated for identical symptoms.
- Radiographic Imaging: Standard periapical X-rays show structural issues, while cone-beam computed tomography (CBCT) provides a 3D view of periapical tissues, helping detect cracks, missed canals, and complex root anatomy that standard X-rays might miss.
- Selective Anesthesia: If there is doubt about the offending tooth, selectively anesthetizing one tooth at a time can help to identify the source of pain.
- Pre-Treatment NSAIDs: Non-steroidal anti-inflammatory medications like ibuprofen may sometimes be prescribed before treatment to reduce inflammation and pain.
- Intrapulpal Anesthesia: For teeth with intense pain (often termed “hot” teeth), injecting anesthetic directly into the pulp chamber after access can provide immediate pain relief.
- Long-Acting Local Anesthetics: Administration of bupivacaine as an anesthetic for post-treatment prolongs analgesia for 6-8 hours following treatment, thus making the immediate recovery of the patient easier.
- Occlusal Adjustment: A slight reduction in occlusal contact of the treated tooth may help to alleviate post-operative pain, especially if the tooth is symptomatic or under heavy occlusal stress.
- Use magnification with illumination: Operating microscopes or loupes offer much better visibility to reduce the possibility of perforation during canal access.
- Radiographs taken before the treatment: A clear understanding of tooth anatomy through radiographs helps in planning the access cavity and avoiding perforation in molars with complex root systems.
- Tactile Awareness: The feeling for subtle changes in resistance when drilling may prevent perforations. If, all of a sudden, the drill encounters less resistance, it may be a signal that a perforation is imminent.
- Ultrasonic Tips: These tools offer fine-tuned control in areas difficult to reach and avoid unnecessary removal of excess dentin to maintain tooth structure and prevent perforation.
- Start with a Conservative Outline: Begin with a smaller access opening, then gradually widen it to provide enough visibility and access without excessive dentin removal.
- Align Access with Root Canal Anatomy: Ensure the access cavity follows the natural angulation of the root canals to reduce stress on instruments and allow straight-line access.
- Remove coronary Obstructions First: Any existing crowns or calcifications should be removed carefully before working on canal access, reducing the risk of canal blockage or perforation.
- Depth Control: Use pre-measured instruments or rubber stops to ensure controlled depth during preparation, preventing unnecessary removal of dentin or accidental perforation.
- Temporary Filling Material: Seal the tooth with a strong temporary filling material like Cavit or IRM to prevent bacterial contamination until the next appointment.
- Occlusal Adjustment: Light occlusion to reduce the trauma of the treated tooth, thereby reducing the possibility of irritation or damage to the interim restoration.
- Intracanal Medication: One may place calcium hydroxide or antibiotic paste within the canal as an intracanal medicament to manage bacterial load and inflammation.
- Pain Management Advice: Please provide clear aftercare tips from the postoperative point of view, including pain management advice, diet, and hygiene to avoid further stress on the treated tooth.
